Sports physicals offered

Total Rehab and 360 Health, 710 Cleveland Ave., Fremont, will provide sports physicals Wednesday, June 14 from 1-4 p.m.
ProMedica personnel will be on hand to provide OHSAA-required physicals for student athletes entering grades 7-12.
The cost is $20, which includes a medical exam, a musculoskeletal exam and a medical history review.
The physicals will serve only as a general screening for health concerns; if a concern is found, students will be referred to their primary care physician.
Time slots are limited and must be paid for in advance. Visit Fremont Total Rehab to pay and pick up OHSAA forms. Cash and credit card payments will be accepted.
An approved OHSAA pre-participation form must be filled out and signed by a parent/guardian before a physical will be given. Parents must be present for all athletes under the age of 18.
