In Oregon Sincera palliative care opens in Toledo Clinic Cancer Center

        To better meet the needs of seriously ill patients across the community, Sincera palliative care has partnered with the Toledo Clinic Cancer Center to offer its services inside the cancer center’s new location at 4330 Navarre Ave., Suite 103, in Oregon.
        Sincera practitioners will be available for face-to-face patient consultations at the Oregon location two Thursday afternoons per month, by appointment only.
        The Sincera team works with community providers to optimize the quality of life for patients living with serious, progressive illness and to prevent hospitalizations or trips to the emergency department. The Sincera team also aids patients, families and referrers by conducting conversations that clarify disease progression, treatment options and goals of care, allowing patients to have control over care decisions and to increase knowledge and awareness of the importance of pain and symptom management among community health care professionals.
        Sincera palliative care is a service started by Hospice of Northwest Ohio in 2008. It has served more than 8,000 patients since that time, with 95% of them reporting an overall positive experience with Sincera.
        In addition to the new location, Sincera’s care is provided in one of its clinic spaces located inside Hospice of Northwest Ohio’s Virginia Clifford Hospice Center in Toledo, the Fulton County Health Center in Wauseon, The Toledo Clinic Cancer Center in Maumee and their Michigan Office in Lambertville.
        How to seek Sincera’s services
        A referral is required before an appointment can be made with the Sincera providers. However, many questions about the program can be answered by calling the Sincera office. A care navigator will work with providers to obtain a referral, if desired. Sincera’s services are covered by Medicare Part B, Medicaid and most other health insurance plans.
